The 2016 Sanitation Seminar with the Illinois Restaurant Association will be held on Wednesday, November 16th from 9am-12pm in the Festival Tent.
Please list all staff members who will be attending the Sanitation Seminar below. All staff members, even those who do not attend the seminar, are required to follow the Food Vendor requirements set out in the Sanitation Seminar. Additional names can be uploaded (in excel format only) in the box below.
The cost for each certificate will be paid for by German American Events, LLC., so please keep your participants to the minimum needed to ensure that at least one certificate holding supervisor is continuously present in the booth.
Failure to follow this rule will result in the revocation of your certificate by the health department.
